YOUTH SOCCER GOALKEEPER WAIVER AND RELEASE OF LIABILITY

This Waiver and Release of Liability ("Waiver") is entered into by the undersigned parent or legal guardian on behalf of the minor participant ("Participant").

1. ASSUMPTION OF RISK
Participation in goalkeeper training, soccer practices, camps, clinics, and related activities ("Activities") involves inherent risks, including but not limited to collisions, falls, ball impact, contact with other participants, weather conditions, and other risks associated with physical activity. The undersigned voluntarily assumes all risks associated with participation.

2. RELEASE OF LIABILITY
In consideration of being allowed to participate in the Activities, the undersigned releases and agrees not to sue Panhandle Goalkeeper Academy LLC, its directors, coaches, volunteers, employees, affiliates, and agents (collectively, the "Organization") from any and all claims arising out of participation in the Activities, including claims arising from negligence, to the fullest extent permitted by law.

3. MEDICAL AUTHORIZATION
The undersigned authorizes the Organization to seek medical treatment for the Participant if necessary and agrees to be responsible for all medical expenses incurred.

4. MEDIA RELEASE
The undersigned grants permission for photographs and videos of the Participant to be used for promotional, marketing, and social media purposes without compensation.

5. GOVERNING LAW
This Waiver shall be governed by the laws of the state in which the Activities take place.
